  • Pediatric Endocrinology of Phoenix (PEP) has closed its private clinic health insurance based services only.
  • PEP will continue the company’s contract and research services.
  • The last day to request insurance authorization was 3/17/17.
  • The last day to request prescriptions was 6/1/2017.
  • It is no longer possible to fulfill prescriptions requests.

  • How do I continue my care?
    • While it is never easy to say goodbye, good medical care is always the priority for our patients.
    • Please schedule an appointment with an endocrinology provider as soon as possible.  This is your responsibility.
    • In the meantime, you can see your primary care provider to help coordinate care, referrals, authorizations etc.
  • Medical Records and Continuity of Care
    • It is the patient and family’s responsibly to schedule an appointment with another endocrinologist as soon as possible.
    • PEP can not be responsible for patients and families who do not schedule follow up care with another endocrine provider, do not request medical record, and/or do not request prescriptions before they are needed urgently by 6/1/2017.
    • Please send medical record and prescription requests in writing to ask@pedendophx.com.  
    • I, Dr Dajani,  will need up to 4 weeks to complete your records when requested before 5/31/2017.
    • I, Dr Dajani,  will need up to 8 weeks to fulfill medical records requests after 5/31/2017  milestone.
    • If you have a more urgent timeline for delivery of our medical records, send details by email (ask@pedendophx.com).
  • Prescription services

    • Dr Dajani will honor prescription requests for patients who have been seen in the past 6 months until 5/31/2017.  
    • Dr Dajani will need up to 5 business days to complete prescription requests.
    • Once you have established care with our new endocrine provider and after 5/31/17,  Dr Dajani will not continue to provide prescriptions to prevent medical errors,etc.

        • OI patients under 21 can be seen; private insurance accepted.Dr Dajani will continue to provide services at CRS for patients who qualify.
