Dr. Dajani and staff have enjoyed being your pediatric endocrinology provider over the past 7+ years and wish you abundant vitality and happiness.
Pediatric Endocrinology of Phoenix has closed its private clinic health insurance based business services but will continue the company’s contract and research services.
The last day of clinic was March 24, 2017The last day to request prior insurance authorizations for medications, procedures, and tests was 3/17/17.

  • Mass notifications were completed in February and March of 2017 by phone, email, text, fax and social media. If you did not receive notice, either:
    • We did not have your updated contact information in our system at your last clinic visit and, thus, we had no way to contact you, or
    • Our messages went to spam or another filter or we could not leave a message
  • How do I continue my care?
    • While it is never easy to say goodbye, good medical care is always the priority for our patients.
    • Please schedule an appointment with an endocrinology provider as soon as possible.  This is your responsibility.
    • In the meantime, you can see your primary care provider to help coordinate care, referrals, authorizations etc.
    • I, Dr Dajani, will personally monitor incoming faxes (602-357-3107) and emails (ask@pedendophx.com); and will provide the following services to previous PEP patients:

      • Medical Records and Continuity of Care
        • It is the patient and family’s responsibly to schedule an appointment with another endocrinologist as soon as possible.
        • I, Dr Dajani,  can not be responsible for patients and families who do not schedule follow up care with another endocrine provider, do not request medical record, and/or do not request prescriptions before they are needed urgently.
        • Please send medical record and prescription requests in writing to ask@pedendophx.com.  I, Dr Dajani, will address emails at this address personally.
        • I, Dr Dajani,  will need up to 4 weeks to complete your records when requested before 5/31/2017.
        • I, Dr Dajani,  will need up to 8 weeks to fulfill medical records requests after 5/31/2017  milestone.
        • If you have a more urgent timeline for delivery of our medical records, please let me,  Dr Dajani,  know by email (ask@pedendophx.com).
          • I will prioritize by urgency, work diligently and as efficiently as is possible.  Please know that I am doing my best while awaiting my superhuman powers to activate. 🙂
      • Prescription services

        • Dr Dajani will honor prescription requests for patients who have been seen in the past 6 months until 5/31/2017.  
        • Dr Dajani will need up to 5 business days to complete prescription requests.
        • Once you have established care with our new endocrine provider and after 5/31/17,  Dr Dajani will not continue to provide prescriptions to prevent medical errors,etc.

    • Complete current PEP research activities at C-103 location in same building.
    • Devote my time to freelance/contract work that does not involve corporate employment, running a business or overseeing a company.
    • Maintain my license, credentialing, business infrastructure and keep the PEP name.
