Do you do HbA1c on site?
Yes, we do finger poke (point of care) testing for HbA1c at your diabetes visit. If it is not covered by your insurance we discount at $15 per test.
Which hospitals do you admit to?
Dr Dajani currently follows and admits patients to Phoenix Children’s Hospital.
May I email the doctor if I have questions between follow-ups?
We are able to answer many simple email questions . Complex medical issues can only be addressed with an appointment. If there is a medical decision involved (for example, concerning prescription for medication), there will be a charge based on how much time the doctor spends discussing it with you. Please keep in mind that our office can receive hundreds of emails daily, and reading and answering takes considerable time for the physician and his medical staff, as well as the office staff, and while we want very much to answer all questions, we must sometimes bill you for our time.
How do I request a refill for my medications?
For established medications, call your pharmacy and request a refill. The pharmacy will obtain approval from your physician. Please allow 48 hours for your request to be completed. For new medications, a doctor’s visit is needed for an evaluation and new prescriptions.
Do you have nutrition or diabetes educator services?
Yes. Our nutritionists and educators are available with your doctor’s visit on certain days per month. Please ask for that clinic when scheduling your appointment.
